Short Description Our interdisciplinary team of experts are ready to provide care and support to both you and your loved ones, wherever you call home. Each physician, nurse and aide is trained and specializes in hospice care, delivering with love and compassion. Additionally, we're able to offer a number of care-related support services: volunteers to provide respite for caregivers, chaplains to offer spiritual support, and social workers to help with practical and emotional needs. Our inpatient facility at Trillium Woods, our bereavement services, with the addition of The Van Andel Hope and Healing Pavilion, and our complementary therapies give us a unique advantage when choosing a hospice agency.
